d) Had any of the following symptoms in the past 24 hrs:

- fever or signs of a fever including chills, sweats, muscle aches and lightheadedness);

-Cough

-sore throat

-pain swallowing

-headache

-runny nose

-unexplained loss of appetite

-diarrhea

-loss of smell or taste
